image The KCHC service delivery model is designed to move the current health system in Kosrae from one that responds to patient-initiated visits for care at a single location to one that much more proactive and moves services oit to the community.  The model responds to the staeady decrement in United States Federal Compact Health Sector funding and in particular addresses  the ctippling cost of caring for patients with complications of NCDs by shifting emphasis strongly toward the most cost effective activities: preventive services and provision of basic primary care.
The KCHC have three main delivery sites, providing 80% of the Kosrae population primary medical, dental, behavioral health, and enabling services within 30 minutes walking distance. Each site have one FTE physician, one FTE nurse, one FTE community health worker (CHW),  and an additional half-time CHWS.  Each site will have weekly well-child, antenatal, prenatal, NCD clinics.  The sites will share one FTE dentist.  Clinic locations are located in Tafunsak, Utwa and Tofol.  KCHC administrative unit is located in Tofol.
